All-on-4 (also known as All on Four) is a revolutionary dental implant technique that allows us to replace a full arch of teeth using just 4 strategically placed implants. Unlike traditional implant methods that require 6-8 implants per arch, All-on-4 achieves the same stability and function with fewer implants.

The two rear implants are placed at an angle, maximising contact with existing bone and avoiding the need for bone grafting in most cases. A full arch of prosthetic teeth is then attached to these 4 implants, providing you with a complete, functional, and beautiful smile in just one day.

All-on-4 is the option I discuss with patients who have lost most or all of the teeth in one jaw, or whose remaining teeth are failing and whose dentures no longer do the job. Four implants support a fixed bridge, the two rear implants are angled to use the denser bone at the back of the jaw, and in most cases a provisional set of teeth is fitted on the same day.

In my experience the planning is where these cases are won. We use 3D scans and digital planning to map the bone and place each implant precisely, and I only confirm same-day teeth once the scan shows the bone can support them. If a staged approach is safer for you, I will say so at the consultation rather than on the day.

My recommendationBring a list of your medications and any recent dental X-rays to the consultation. Both change how I plan an implant case, and having them on day one saves weeks.

Questions patients ask me

Will I really leave with teeth on the day?

For most patients, yes. A provisional fixed bridge is fitted the same day and replaced with the final bridge once the implants have healed, which usually takes three to six months. I only promise same-day teeth once your scan confirms the bone can support them.

What if I have been told I do not have enough bone?

That is often the reason people ask about All-on-4 in the first place. Angling the rear implants makes use of the bone that is usually still there, which avoids grafting for many patients. Where a graft is unavoidable, I explain the extra stage before anything starts.

How long will it last?

The titanium implants can last a lifetime with good care, and the bridge on top typically needs replacing after ten to fifteen years through normal wear. Cleaning under the bridge daily and keeping to your review and hygiene visits is what protects it.
